Rehabilitation Therapist Requirements:
- Graduated from an accredited Physical Therapy (PT) college or university.
- Hold an unencumbered Physical Therapy (PT) license for New York.
- Conduct home visit Physical Therapy assessments of referred patients to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
- Proficient in EMR documentation (experience with Hello Note preferred).
- Current CPR Certification.
