Responsibilities
- Conduct home visit assessments of referred patients, providing direct patient care to assess medical conditions and rehabilitation potential.
- Proficient in EMR documentation.
- Maintain an unencumbered Physical Therapy license for New York.
- Follow CPR Certification requirements.
Requirements
- Graduated from an accredited Physical Therapy college or university.
- Experience with EMR documentation (Hello Note system knowledge is a plus).
- Flexible scheduling options available.
- Ability to relocate to Brooklyn for in-person work.
