Rehabilitation Therapist Requirements:

• Graduate from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license in New York.
• Conduct home visits to assess patients, evaluate conditions, and develop rehabilitation plans.
• Experience with EMR documentation is a plus.
• CPR Certification.
